The vision is to create a ‘person centred system where a person with a disability, their family and/or carers are able to determine their support and community access arrangements’
METS is offering the opportunity to investigate some of these ‘Person Centred Thinking Tools’ to see how they can be used effectively within the context of the current working environment.
We will be looking at using and building:
• One page profiles
• Sorting, What’s important to/for
• What’s working /What’s not working
• Learning logs
• Communication charts
